modal dialogs

Oddly, today modal dialogs have popped up twice on a live web server.

The first was due to a path error. The error wasn't Panorama's fault, 
but it should not have produced a dialog.

The second  said roughly;  "Explicit assignment to a cell is disabled".

Of course, each one brings web serving to a halt.

Since the explicit dialog has never appeared before - and this set of 
files has run for quite a while - what sort of processes should I be 
looking for to find it's cause?
