I¹m having the exact same problem.
A file set that I¹ve been using for years in 3.1.5, two simple dbs, the
first has a very simple two command .Initialize procedure (Field and GoForm)
Thanks to Elon for diagnosis, I¹ve not had time to troubleshoot, and since
clicking the file set icon a second time opens the second db, I¹ve been
working with it as is.

In each of the examples the DBs are simple 3 field, only a data sheet with
empty cells and when they fail a simple .initialze procedure with exactly
one non comment item like
local test
Absolutely nothing more. And Panoram 5.5 has been re-installed . Mac
10.4.11, G5. The examples are on the same HD as the System and the Panorama
software.
When the .initialize procedure was removed from the Address DB there was no
load problem. The Address DB has been running thru 4.x and I believe 5.0.
Remove that procedure from the examples and no problem.
It may, indeed, be a problem local to me, but the examples that fail are
very simple and very repeatable.
