It can easily be 100% local. If you are using the domain only on your
LAN, and perhaps WAN, you don't have to go to a registry or even have a
registered domain. You can make up your own domain names. For all my
domains for testing before going public I have identical domains set up
as my public domain except for the suffix. The local suffix I have
chosen is .dev and the configuration is set so that domain.COM goes out
to the Web and domain.DEV stays local.
Here is a link to a tutorial I wrote using Abyss web server software.
The basics would apply to other servers and the editing of the local
hosts file is one of the most important points.

You can also set up a DNS program such as DNS Enabler (only $15) and
keep specific domains totally within your network. This may be the
easiest method.
